Service Objectives
Our Centre provides different types of individual, family and community support and care services to assist elders to develop their potential, to maintain satisfactory family and personal relationships, and to solve individual and family problems, thereby enabling old people with different levels of functioning to continue to live in the community they have known well.
Service Contents
We provide home-based care services that include: professional nursing, rehabilitation training, personal care, meal delivery, nursing care, household cleaning, shopping and escort service, etc. (Those requiring enhanced nursing care would be required to go through an assessment by Standardized Care Need Assessment for Elderly Services operated by the Social Welfare Department)
Service Target
Elders, and persons with sicknesses, disabilities and special needs (with no age limit) who are living within the service boundaries listed below. Priority would be given to those with financial difficulties or unfavorable circumstances.

Choi Hung Estate、Choi Wan Estate、Choi Fai Estate、Choi Fung Court、Sun Lai Garden、Scenic View、Fung Shing Street Disciplined Services Quarters、Ngau Chi Wan、Tsz Ching Estate (Ching Fai House/Ching Tak House/Ching Wo House/Ching Yi House/Ching Yuk House)、Tsz On Court、Tsz Man Estate
Application of Services
Persons in need or their family members/relatives can call or approach our Centre in person to enquire.
All applications will have to go through an assessment by our social worker.
Fee
Fees will be determined subject to an assessment of the family financial circumstances of the applicant.
Opening Hours
For Regular Services:
Mon to Sat 8:30am to 6:30pm
Sun & Public Holiday Close
For special arrangement on every Sunday, Public Holiday and irregular services:
After the need assessment, special arrangement of services will be arranged to the service users.
