Professional Outreaching Team for Private Residential Care Home for Persons with Disabilities (Team 1)

Service Objectives

The scheme aims at strengthening the social and rehabilitation support for Persons with disabilities (PWDs) who are residing at the private Residential Care Homes for Persons with Disabilites (RCHDs) and providing training and support for their family members/carers as well as the staff of the private RCHDs through the provision of a package of multi-disciplinary rehabilitation services. Specific objectives of the Scheme include:
  1. Providing a package of social work, paramedical and nursing support services for residents of private RCHDs to meet their social and rehabilitation needs;
  2. Providing consultation, training and support for the family members/carers of residents and staff of the private RCHDs for betterment of care and support to the residents; and
  3. Promoting social inclusion for residents of private RCHDs through community networking activities/programmes with other stakeholders in the community.

  1. Occupational therapy, physiotherapy, speech therapy
  2. rehabilitation training
  3. nursing services
  4. consultation services
  5. psychosocial support activities/programmes
  6. Clinical psychologist support
  7. Carers support
  8. RCHDs staff support

Service Target

Persons with disabilities who reside at private RCHDs located in Tsuen wan and Kwai Tsing District.

RCHDs in Tsuen Wan and Kwai Tsing District

Application of Services

Self-approach by PWDs. Carers and staff of RCHDs can refer the PWDs for assessment as well. Service Manage will contact the PWD or the referrer for further assessment and confirm the date of service delivery.

Withdrawal of Services

  1. The service user is discharged from the private RCHD due to various reasons, e.g. admission into subvented residential care service, death, home restoration, etc.,
  2. The service user has been hospitalized for a period of more than three months without a specific discharge plan; or
  3. The service user decides to terminate the service.

Fee

Free of charge for on-site treatment/training and psychosocial activities

Opening Hours

Mondays to Saturdays: 8:30 a.m. to 5:30 p.m.
Lunch Hour: 1pm to 2pm
(Closed on Sundays & Public Holidays)
