** Our agency’s Integrated Home Care Services in the Wong Tai Sin and Sham Shui Po locations have participated in the Pilot Scheme on Community Care Service Voucher for the Elderly (CCSV) and can accept service users from the following districts:

1. Kowloon West – Tsim Sha Tsui, Kowloon City, Sham Shui Po (Note: Our unit also accepts cross-boundary service users);
2. Kowloon (East) – Wong Tai Sin (Note: Our unit also accepts cross-boundary service users).

Professional Nursing Care and Rehabilitation Plan

Special Characteristics:

Based on the needs of each service user, we will arrange a home visit and professional assessment by members of our nurse/physiotherapist/occupational therapist, registered social worker team to design an appropriate nursing care and rehabilitation plan for the servicer user.

A Customized Professional Nursing Care and Rehabilitation Plan could include: 

* Rehabilitation Program for Stroke Patients;
* Care for patients suffering from Diabetes, including Nutrition and Skin Care;
* Cognitive Training for patients suffering from Dementia;
* Rehabilitation Support Plan for patients upon being discharged from hospital;
* Pain/Cancer Management Plan;
* Home Safety Improvement Plan

Application of Services

Withdrawal of Services

Withdrawal can be made in person at our Service Centre, or by phone to speak with our staff member on duty or to our Social Worker who is responsible for handling the service withdrawal process.

Opening Hours

Service Hours for Home Care and Daycare Services:

Home Care Service: 6:00 a.m. – 10:00 p.m. (Minimum for a service session = 2 hours)
Daycare Service: Mondays – Saturdays, 8:00 a.m. – 8:00 p.m. (Half Day/Full Day)
