Yau Mong Integrated Home Care Services
Service Objectives
Provide home care service, nursing care and rehabilitation services in the Yau Mong District for the elderly people who lack the ability to look after themselves, persons with disabilities, and families in need by assisting them to handle problems in their daily living, and enable elders to continue to live in the community they know well.
Service Contents
Provide home-based support services: professional nursing care, rehabilitation training, personal care, meal delivery, supervision, household cleaning, shopping and escort service, etc. (To use the Enhanced Home and Community Care Services, it is necessary for the service applicant to be assessed by SWD through the Standardised Care Need Assessment Mechanism for Elderly Services)
Service Target
Elderly persons, persons with sicknesses, disabilities or in need (irrespective of age), and persons requiring financial support, who are residing within the service boundaries described below.

Application of Services
Applicants can enquire by phone or in person. They can also seek help from any social service agency to make a referral to our service unit. All applications must go through an assessment by our social worker.
Fee
Fees are determined based on financial situation of the applicant.
Opening Hours
For Regular Services:
Every Monday to Friday, 8:45am to 6pm
Every Saturday, 9am to 12:45pm
For special arrangement on every Sunday, Public Holiday and irregular services:
After the need assessment, special arrangement of services will be arranged to the service users.
